Understanding Folliculitis

Folliculitis is an inflammatory condition of the hair follicles, most commonly caused by bacterial infection (Staphylococcus aureus), fungal overgrowth, or physical irritation. It affects people of all skin types and can occur anywhere on the body where hair follicles are present — including the scalp, back, chest, arms, and legs.

The condition is characterized by small red or white-headed bumps, pustules, itching, tenderness, and in more persistent cases, crusting or scarring around the follicle. Folliculitis is often mistaken for acne, but unlike acne, it is specifically centered around individual hair follicles and can be triggered by a distinct set of factors:

  • Bacterial infection – most commonly Staphylococcus aureus, which colonizes the skin surface and enters follicles through minor abrasions
  • Fungal overgrowth – particularly Malassezia on the scalp and upper body
  • Shaving and friction – razor bumps (pseudofolliculitis barbae) are a form of folliculitis caused by ingrown hairs
  • Occlusion and sweat – tight clothing, heavy moisturizers, and prolonged sweating can block follicles and create an environment for bacterial growth
  • Hot tub folliculitis – caused by Pseudomonas aeruginosa in poorly maintained water
  • Compromised skin barrier – eczema, dry skin, or frequent use of harsh cleansers can increase susceptibility

Folliculitis is one of the most common skin conditions seen in dermatology. It affects men and women equally and is more prevalent in people who shave regularly, athletes who sweat frequently, individuals with diabetes or weakened immune systems, and those living in hot or humid climates. It can be acute (short-term) or chronic (recurring over months or years).
